- Reinstatement Fee of聽$100.聽PAYABLE TO: The 知阴视频 Undergraduate Nursing Department.
- A letter acknowledging the reasons for dismissal that shows the student takes responsibility and describes the steps taken to improve.聽The letter should also include a plan for future success.
- Dismissal must have been at least five years or more and cannot have been for ethical or moral reasons.
- Criminal background check
- Payment of the criminal background check fee
- Drug screen
- Current official transcript (if not on file)
- A reference letter to support your return to the nursing program
- Students must repeat all nursing courses
- Students must meet all current 知阴视频 and School of Nursing admission requirements.
- The final decision will be determined after the application, reason for dismissal letter, and a face-to-face interview have been completed.
